I found their "Citizenship Correction Form" to be sent to LB Bork at the Address below. It has all of the hallmarks of a scam.
Choice quotes:

Really? Something like that should be backed up in your order form with a citation of the U.S.C. or Supreme Court ruling.

THE FALLACY

The American Union is actually a federation of the sum of American countries; and the-sum of-states are entitled the United States of America-the State of the federation is referred to as The United States of America. What this process does is legally terminate the nationality and citizenship that is caused by the Fourteenth Amendment and establishes the lawful nationality of your true country.
INSTITUTING THE PROCESS
To provide a basic background of how the federal government functions, the United States Congress (federal legislature) is qualified to make laws pursuant to the Constitution of the United States. Generally, the Law that Congress can establish encompasses two bodies:Public and Private. In general, Public Law deals with the Constitution and criminal matters;Private Law deals with things that encompass matters of private nature-that is-it does not apply to everyone-some kind of voluntary duty-or some other-has to be established-such as creating a private corporation under laws of a particular State or the United States. In due course, the Fourteenth Amendment is fundamentally private law because it alters the original premises that were fixed under the Constitution, therefore, the citizenship that is created by the Fourteenth Amendment is voluntary, hence, can be terminated.
As it has been established that the legislature creates the law, the specific department of the government that executes the law is the Executive; as you are aware, the Executive department of the United States is held by the President of the United States. Under this particular department are several agencies. These agencies are the ones that you will be dealing with when you execute the 'administrative' process. The legal process involves a procedure, which is not much different than filing a tax return. As Congress has established federal statutes that require persons to file income taxes (which is private law), Congress has also provided statutes to carry out this process (which is public law). If you have any doubts that LB Bork is running a legitimate business, this should put it to rest. Lately, this form isn't easy to find without Google, his website now requires that you make an appointment with him. He probably uses the same form, just like Scientology still uses the same material even though the whole world knows about OT III.
You see, this is where I screwed up. I should have reported him to the FBI before airing out his dirty laundry. I shouldn't have given him a chance to clean himself up (via RJ Mills relaying the information, or him Googling about himself).
Also, I wish I was a bit smarter at translating. It's pretty obvious.
